Gary, the readings I have are on cassette tape, published (as far as I can tell) by the Poetry Room at Harvard College Library. VN's reading of a verse excerpt from Pale Fire took place on April 10, 1964. He read the sections from Canto Two concerning Hazel's death. My "Genius and Plausibility" piece goes into somewhat excruciating detail about this.
I don't know if this cassette collection has been digitized and made available online. It certainly ought to be -- VN's readings and occasional ad libs are delightful.
Best,
J.
